Pregnancy is a time of change.  Every woman can use help with planning for a healthy pregnancy, getting ready for a new baby, and the lifestyle change that a new baby brings to the family.  PNCC or Prenatal Care Coordination is a Medicaid benefit that helps pregnant women get the support and services they need to have a healthy baby.  A care coordinator will listen to your concerns and answer your questions.

The care coordinator will help you by asking about your:

  • Current and Previous Pregnancy
  • Health History
  • Relationships
  • Worries

A care coordinator will work with you one on one to provide a personalized, confidential plan of care. PNCC services include:

  • Help Getting the Health Care You Need
  • Personal Support
  • Information on Good Eating Habits and Health Practices
  • Information and Help Finding Needed Services in Your Community
